Mary
I have a daily, weekly, and monthly "To Do" List which I add to and subract from as needed. I have both a "personal to do" list and a "business to do" list. At the beginning of each month, I block out the specific items that I know I have committed to. Then at the beginning of each week, I make the necessary modifications depending on various factors. Each day, I have a specific "to do" list of tasks to be completed and I cross them off as I do them. (Gives me a sense of accomplishment). I use the technique on client jobs as well, and any other projects I'm working on. I break things down into smaller pieces, while paying attention to the longer term goal I want to achieve.
I stay focused by telling myself to "PAY ATTENTION!" and I do not attempt to multi-task too much. I may have 2-3 things going at the same time, but try to remain focused on the thing I want to accomplish in a given time frame. I also gave up being a "perfectionist" many years ago. I think that came with maturity and the wisdom I gained as I got older.
